| Paradigm Family Medicine will submit a bill to your insurance company. It is, however, the responsibility of the patient to determine whether the services rendered are covered benefits. Co-payments and full payment for services not covered by your insurance company are required at the time of service. Furthermore, if your deductible has not been met by the time of service you may be responsible for full or partial payment to fulfill your deductible. We do accept payment in the form of cash, check or credit card. The financial policy details are available on this website. | |
