Paradigm Family Medicine is a preferred provider in many insurance networks.  We do provide billing services for patients with insurance.  We utilize a billing service to process medical claims and manage patient accounts.  Our contractual obligation is to collect copayments and co-insurance amounts at the time of service.  You, our patients, are required to present your insurance card at every visit.  Copayments must be collected at the time of service.  We ask that you update your insurance information as soon as a change occurs to avoid delays in billing.  Failure to do so will result in the charges being transferred to your account with payment due immediately. 

Paradigm Family Medicine does not perform third party billing.  In the event of an accident, if you wish to be seen at Parardigm Family Medicine, you will be responsible for all charges incurred at the time of service.  We will provide invoices which you can file with your insurance company.  We do not accept liens.

Paradigm Family Medicine does not re-open closed Labor and Industries claims. We provide limited services for on-the-job injuries. It is essential that such injuries be identified at the time the appointment is made so that proper documentation is completed.
